Hmm.
There are some things you can't do much about, but ... Writing an Awful Query Letter(TM)?
Submitting a manuscript to an agent who doesn't represent your type of book? Not following the guidelines? Reject. *STAMP* 
"Not polishing your writing" is another good one. Sure, no-one's going to buy a story with spelling mistakes etc.
Trouble is, where do I find an agent?
I've googled etc. and can't find one - maybe my google-fu is weak?

Actually, the best way to research the agent for you is to read current comp sales (you have to know what the market is doing), once you find one, read the acknowledgements. Authors often mention their agent, then Google their name.
